If you believe we have failed to provide services or discriminated in any way, you may file a grievance in person, by mail or by calling (503) 666-2196. Please include your name, contact information and a description of the issue.

Review

Our compliance coordinator will investigate and respond in writing within 30 days. You may also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights.
